PERMANENT HOUSING DISABILITIES – PHD
A permanent housing program, that provides rental assistance and weekly case management for single homeless men and women, and homeless men and women with families, with permanent disabilities. 

An individual is interviewed, and if accepted, placed in an apartment in the community.  The individual pays a portion of the rent based on income with the program paying the difference.  The program covers the security deposit and all utilities.  A goal plan is developed, and individual meets with the case manager weekly who monitors progress with goals. Goals may focus on maintaining housing, improving financial situation, maintaining health, obtaining part time employment, pursuing higher education, etc.

Individuals are assisted financially until they receive a section 8 voucher, public housing, have change in income, which allows them to be self sufficient.  The individual may continue to receive case management services after financial assistance ends.
